Here's an example PDF form that I created in OpenOffice; http://pirateparty.net.nz/files/application.pdf
I am able to enter text in the fields in evince. I don't know about saving/submitting though (because we chose not to do any of that) but as near as I can tell evince supports PDF forms just fine.
